Amazon Plan of Action templates for the five most common deactivations
A Plan of Action is graded on three things: root cause, corrective action, and preventative measures. The templates below cover the five triggers behind most deactivations. Adapt one to your situation rather than sending it as-is, because a reviewer recognises boilerplate immediately and a generic plan is the most common reason a legitimate appeal fails.

The rules that apply to every Plan of Action
- Use the three headings literally. Root cause, immediate corrective actions, preventative measures. Reviewers scan for them.
- One page. Bullets, dates, specifics. Length is not thoroughness, and long plans get skimmed.
- No emotion, no blame. Nothing about how important the account is to you, and nothing blaming buyers, couriers, or Amazon. None of it is scored.
- Past tense for what is done, present tense for what is running. Promises about the future are the weakest content you can include.
- Attach evidence Amazon can verify. Invoices from contactable suppliers beat any wording you could write.
Inauthentic or not-as-described complaints
These turn on your supply chain, not your process. The plan matters, but the invoices decide it.
ROOT CAUSE I sourced [ASIN / category] from [supplier], a distributor I had not verified as an authorised source for the brand, and I had no incoming inspection step before listing. Units that did not match the ASIN detail page reached buyers, producing [N] complaints between [date] and [date]. Intellectual property complaint
The important thing here is that Amazon is not the decision maker on the underlying rights question. A retraction from the complainant resolves the case far more reliably than the strongest plan you could write, so pursue both at once.

Related or multiple accounts
If the deactivation is about a link to another account, a performance plan is irrelevant. What Amazon needs is the relationship explained and the link resolved.

The connection exists because [state the truth plainly: shared address, shared bank account, a business partner, a family member selling from the same premises, or a previous account of my own]. Common questions
How long should a Plan of Action be?

About one page. Root cause in a short paragraph, then bullets under corrective actions and preventative measures. Reviewers work at speed, and additional length pushes the graded content out of view rather than strengthening it.
Why does Amazon keep rejecting my Plan of Action?

Most often the root cause is a symptom rather than a systemic failure, or the preventative measures are intentions rather than controls that are already running. Rejections are usually templated, so they will not tell you which section failed. An Account Health Specialist call often will.
Should I apologise in a Plan of Action?

One clear acknowledgement of responsibility is enough, and it belongs inside the root cause. Extended apology does not affect the outcome and takes space from the content that is actually scored.
Can I reuse a Plan of Action that worked before?

Only as a structure. The facts, dates, metrics, and controls have to match the current deactivation and your real account data. A plan that does not line up with what Amazon can see is worse than no plan.
